Founded in 2010, and joining the CBC family of providers in 2021, we set into motion the personal beliefs that individuals cannot fail at living in the community when they are offered the support they need. Nearly 15 years later, we’ve grown and are proud to be able to assist even more of our Florida community!
Florida I/DD services and supports, tailored to your needs.
We provide our Florida community with I/DD Intermediate Care Facility (ICF) residential services, with 14 home locations in 10 different FL counties in Central, Northeast and Central Florida APD regions. When it comes to individuals with Intellectual and Developmental Disabilities (IDD), an ICF is a specialized residential facility that provides a higher level of care compared to group homes or assisted living facilities. To qualify for IDD ICF services an individual must have an IDD diagnosis prior to age 22 and active Medicaid.
- Specialized Care: ICFs are designed to meet the specific needs of individuals with IDD who require ongoing support and assistance with daily living activities, such as personal care, medication management, and therapy services.
- 24-Hour Supervision: Residents of ICFs receive round-the-clock supervision from trained staff members to ensure their safety and well-being. This continuous support is essential for individuals with IDD who may have medical or behavioral challenges.
- Individualized Treatment Plans: Each resident of our homes has a personalized treatment plan tailored to their unique needs and abilities. This plan typically includes goals for skill development, behavior management, and health maintenance.
- Multidisciplinary Approach: ICFs employ a team of professionals, including nurses, occupational and physical therapists, behavioral specialist, and direct care staff, who work collaboratively to provide comprehensive care and support to residents.
- Community Integration: While ICFs offer a structured and supportive environment, SNO GHH also emphasizes community integration and meaningful day activities. Residents enjoy social activities, community based skill building and recreational opportunities within a home and community based setting.
- Regulatory Oversight: ICFs for individuals with IDD are licensed and must adhere to state and federal regulations governing residential care facilities. This ensures that residents receive quality care and that the home meets safety and licensing standards.
Overall, ICFs play a vital role in providing long-term care and support for individuals with IDD who require a higher level of assistance than can be provided in other residential settings. They offer a safe, structured, and nurturing environment where individuals can thrive and reach their full potential.
