Michael Kotzen

Michael is the President of Community Based Care, LLC (“CBC”) and is also a member of CBC’s Board of Directors.
Michael is responsible for developing and implementing the overall strategy of CBC while ensuring the delivery of the highest quality service to CBC’s clients and families. Michael is driven to expand CBC’s reach so that more clients can benefit from CBC’s promise of customer focus, commitment to quality and dedication to its well- trained, outstanding staff.
Michael brings more than twenty-five years of diverse healthcare leadership experience. He earned a Bachelor’s degree in Business Administration from Rutgers University and a MBA from St. Joseph’s University. Michael is a fellow in the American College of Healthcare Executives.

Laura Fortier is the Chief Human Resources Officer of Community Based Care (CBC).
As Chief Human Resources Officer, Laura is responsible for the oversight of all aspects of Human Resources including leadership development, workforce planning, talent management and planning, compensation and benefits and employee relations. Laura provides strategic leadership to HR ensuring alignment of strategies with the mission, vision and values of the Community Based Care family of providers.
Prior to joining CBC, Laura has served in HR in a number of leadership capacities during her career including healthcare, distribution and non-profit organizations.
Laura earned both her Bachelor of Science in Management and Master of Business Administration from North Carolina State University.

Michael Vallejo is the Chief Information Officer of
Community Based Care (CBC).
Michael is responsible for developing and implementing the technology strategy for CBC which enables our caregivers to most efficiently achieve our mission to support the best health and fullest lives for the people we serve.
Michael has more than 20 years of experience in the Information Technology industry, focusing on innovation and process improvement. His experience has included several sectors such as manufacturing, state and federal government, but more than half of his career has been in healthcare. Prior to Community Based Care he was the CIO at Care Hospice, one of the fastest growing hospice providers in the country.

Laura Hayes

Vice President of Quality, Safety, Performance Improvement
Laura Hayes serves as Vice President of Quality, Safety & Performance Improvement for Virtua. In this role Laura is responsible for overseeing and directing the development of a comprehensive quality and patient safety program which includes the development of the hospital’s quality strategy, annual goals, quality incentive programs and providing direction to the Quality Department, Infection Control, Clinical Outcomes Council and Patient Safety Committees. The role also is responsible for coordinating and directing activities related to The Joint Commission accreditation, continuous CMS readiness, NJ State requirements, as well as ensures compliance with all aspects of performance improvement.
Prior to her current role, Laura was Director of Quality/Risk Management for 10 years at Virtua Voorhees Hospital. During her time as Director of Quality, Virtua Voorhees was recognized by the Leapfrog Group as a “Top Hospital” for excellence in quality and patient safety in 2014, 2015, & 2016.
Previously, Laura has held various roles including staff nurse on MS and ICU units as well as adjunct facility positions. Laura started her career at Virtua in September 2000 as a Clinical Nurse Specialist at Virtua Memorial and was promoted in August 2003 into the role of Nurse Director.
Laura earned a Bachelor of Science in Nursing and a Master of Science in Nursing Education from Wagner College and holds a Certification in Healthcare Quality.

Gene Rodgers is the Executive Vice President of Strategy & Corporate Development of Community Based Care (CBC), as well as a member of the CBC Board of Directors.
Mr. Rodgers provides the overall strategy for the agency as well as focuses on marketing, business development, rapid growth, and managing multiple acquisitions through the closings.
Mr. Rodgers leads the marketing and business development teams in North Carolina and surrounding southeast states. He is personally involved in the acquisitions from identifying a target, to valuation and all the way through to closing working with financial and legal professionals and executing the deal.
Mr. Rodgers has a Bachelor’s degree in Social Work from UNC- Wilmington, and a Master of Social Work degree from the University of South Carolina. He is a veteran in the field of behavioral health and, more specifically, services for the Intellectual and Developmentally Disabled (I/DD). Since 1992, Mr. Rodgers has risen to progressively higher executive level positions in the industry.
Since 2000, Mr. Rodgers has been specifically focused on business development in the I/DD and Behavioral Health private provider space through both organic and acquisitive growth strategies.

Jennifer Vogt is the Senior Vice President of Operations for Home Care Services.
Jennifer is responsible for developing and implementing the strategic plan for all Home Care Services across CB Homecare and its affiliates. CB Homecare aims to achieve the highest possible level of service for all customers: clients, families, case managers and the community. Jennifer strives to expand the CB Homecare footprint across state lines in order to provide care to more clients in need while staying in their homes.
Jennifer joined Community Based Care, LLC in 2021. Jennifer has more than twenty years of leadership experience in the Home and Community Based sector. She earned a Bachelor’s degree in Nursing from the University of Texas Health Science Center, San Antonio and an MBA from The Pennsylvania State University. Jennifer is a Board Certified Registered Nurse.

Wendy Steele

In this role, Wendy is responsible for operations across our IDD operations in North Carolina and Virginia.
As Vice President of IDD Operations, Wendy overssess all Home and Community based waiver services as well as all group home and Intermediate Care Facilities (ICFs) for NC and VA. She is passionate about delivering high quality services and customer service to the individuals and families that we support.
Wendy has over 22 years of experience in the IDD field, beginning her career of service as a Direct Support Professional at Lindley Habilitation Services and has since held various progressive leadership positions within Lindley before joining CBC. Upon joining the CBC family Wendy served as CEO for CBC Lindley and later her role expanded as CEO of CBC Pinnacle. Most recently, Wendy has been responsible for all IDD operations across eastern North Carolina.
Wendy earned her Bachelor’s Degree in Business Administration from the University of North Carolina, Greensboro.
